What is Case Management?

Case management for pre and non-NDIS people involves bringing service providers together and coordinating supports during a life transition for someone with disabilities or mental illness. This may be an older Australian over 65 who does not qualify for the NDIS, a young person transitioning out of child safety, or someone with mental illness in hospital transitioning back into the community. We work closely with our Government funders to ensure the right care and supports are in place for a smooth and safe transition.
